Align Technology posted adjusted earnings per share of $2.80 for Q3 2026, beating expectations for a fourth straight quarter on cost discipline while the top line stayed under pressure.

North American orthodontist shipments rose year over year for the first time in six quarters, the clearest sign yet that the Invisalign demand slump is finding a floor. CEO Joe Hogan said DSO shipments were up double digits across orthodontist and GP customers.

Clear Aligner cases shipped grew 6.6% year over year, with record doctor submitters offsetting flat gross margin and a continued decline in the Imaging Systems and CAD/CAM Services segment.

Shares fell after the report despite the beat, a sign the good news was priced in and caution around discretionary healthcare persists. Management guided Q4 revenue to roughly $1.0 billion to $1.02 billion and plans to repurchase up to $300 million of stock in the quarter.

Align also announced a U.S. pilot of Invisalign Sports Guard, a custom 3D printed mouth guard made from an iTero scan. For 2027, management guided to another year of revenue growth and about 100 basis points of operating margin expansion, with no consumer recovery assumed.
