Becton Dickinson Q4 2026: Beat on Both Lines, Free Cash Flow Up 42.5%
As seen on the 24/7 Wall St. homepage on November 5, 2026.
The first full quarter of the slimmed down MedTech company beat on both lines, and management guided fiscal 2027 adjusted EPS to $13.10 to $13.30 against $12.70 last year. Free cash flow jumped 42.5%, the tell that the Life Sciences spin is paying off.

Becton Dickinson's fiscal fourth quarter was the first full reporting period since the February 2026 spin-off of its Biosciences and Diagnostic Solutions business, and the slimmed-down MedTech company used it to demonstrate that the restructuring is working. Adjusted diluted EPS came in at $4.15 against a consensus estimate of roughly $4.07, a beat of about 2%. Revenue of $5.27 billion topped the $5.11 billion estimate by 3.14%, extending the company's run of consecutive EPS beats to four straight quarters.
The reported year-over-year revenue decline of about 10.5% is largely a bookkeeping artifact of the spin-off, with all figures presented on a continuing-operations basis that excludes the divested Life Sciences unit. On that apples-to-apples basis, growth was broad: Interventional rose 7.6% and BioPharma Systems gained 6.5%, while Advanced Patient Monitoring posted U.S. revenue growth of 20.5% and international Medication Management Solutions grew 11.4%.
Free cash flow of $2,933 million for fiscal 2026 represented a 42.5% jump, the clearest evidence yet that stripping out the Life Sciences segment improved the underlying cash generation profile of the remaining business. Operating income for the quarter came in at $1.03 billion, up 48% year over year on a continuing-operations basis, though results did include a $66 million IEEPA tariff refund that provided a modest GAAP tailwind.

Looking into fiscal 2027, management guided adjusted diluted EPS to a range of $13.10 to $13.30, compared with the fiscal 2026 figure of $12.70, implying low single-digit revenue growth. CEO Tom Polen described the company as entering the new year from a position of strength, citing what he called a portfolio built for durable performance. BD also announced a 2026 Investor Day on December 10 at the New York Stock Exchange, where it is likely to lay out longer-term targets for the newly configured business.
One area worth monitoring is U.S. Medication Management Solutions, which declined 5.6% in the quarter even as the international side of that segment grew. Tariff policy volatility and China volume-based pricing pressure were also flagged as ongoing risks in the filing. The company's planned $19 billion U.S. investment over several years, including $3 billion earmarked for manufacturing expansion, shapes the near-term capital allocation picture even as free cash flow has improved sharply.
