Glaukos posted adjusted EPS of $0.47, beating expectations and turning adjusted profitable for the first time in eight quarters.
Two launches flipped the model: iDose TR, the intracameral glaucoma drug implant that lifted U.S. Glaucoma revenue 47%, and Epioxa, the epithelium-on corneal cross-linking therapy whose July 1 J-code activation accelerated reimbursement.
Glaukos swung to GAAP net income of $2 million from a year-earlier loss, with non-GAAP gross margin expanding as both launches scaled.

Management raised full-year 2026 net sales guidance and introduced preliminary 2027 guidance of $1 billion, assuming growing contributions from iDose TR, Epioxa, and iStent infinite. Risks include Medicare coverage reviews for iDose TR and physician fee pressure on cataract and MIGS procedures.
Whether the inflection holds depends on how the LCD review resolves and whether Epioxa's reimbursement momentum continues as it expands toward sites covering more than 95% of the U.S. population.
