HCA Healthcare's Q3 2026 adjusted earnings of $7.22 per share came in ahead of expectations, with revenue also beating estimates. Those beats landed while the company was absorbing one of the more punishing headwinds in the hospital sector this cycle.

The central challenge of the quarter was a sharp payer mix deterioration: patients losing coverage on health insurance exchanges, driven by administrative reforms and the expiration of enhanced premium tax credits, pushed uninsured volumes higher. Management estimated the shift cost roughly $400 million in pre-tax income. Margins narrowed as that uninsured surge pressured pricing.

Volume trends offered a partial offset, with same-facility admissions, equivalent admissions and emergency room visits all rising. Aggressive share repurchases also cushioned per-share results: the company bought back 7.23 million shares in the quarter alone.

With the quarter behind it, management raised its effective outlook by narrowing expectations to the upper half of prior full-year guidance ranges. The guidance update signals that executives believe the coverage-loss headwind is manageable rather than worsening.

CEO Sam Hazen pointed to scale and what he called a disciplined culture as the foundation for that confidence, noting that the company believes it is positioned to end the year positively despite the federal policy changes that reshaped the coverage landscape in 2026. The company also declared a quarterly cash dividend of $0.78 per share, payable December 29 to shareholders of record as of December 15.
