Medtronic delivered adjusted EPS of $1.45 for its fiscal first quarter of 2027, beating expectations for a fifth consecutive quarter. Revenue grew 13.7% year over year, landing above the company's own guidance midpoint.

Management raised its full-year organic revenue growth outlook to a range of 7.25% to 7.75%, up from 6.75% to 7.25%, and tightened non-GAAP diluted EPS guidance upward. A beat paired with a forward guidance lift signals durable momentum.

The Cardiovascular Portfolio led the quarter with revenue up 19.5%, powered by Electrophysiology Therapies and strong commercial momentum behind the Affera Mapping and Ablation System. The Affera system also received an expanded CE Mark indication for the treatment of ventricular arrhythmias during the quarter, broadening its addressable market.

Strength was not confined to cardiovascular: Neuroscience, Medical Surgical and Diabetes all grew double digits, with Diabetes up 16.9% on insulin pump and continuous glucose monitoring demand. The breadth reduces the risk that any single segment slowdown derails the full-year outlook.

Operating cash flow more than doubled year over year to $1.79 billion, and free cash flow grew nearly 121%. Medtronic kept returning capital through dividends and share repurchases even as it plans a separation of the Diabetes business through capital markets transactions.

An extra fiscal week in Q1 contributed approximately $570 million to organic growth, which inflates the year-over-year comparison. Investors focused on the sustainability of the 13.7% top-line growth rate will want to track how the remaining quarters perform as that calendar tailwind fades from the base.
