GAAP net income fell 52% to $38 million, so the headline beat matters less than the margin pressure behind it. Molina reaffirmed full-year adjusted EPS of at least $5.25, and CEO Joseph Zubretsky said 2027 rate updates may mark the beginning of the turn in Medicaid margins. Sellers moved first anyway.

Molina Healthcare beat Wall Street's third-quarter bar on both lines, with adjusted EPS of $0.89 exceeding expectations, a result that looks clean for a managed care company navigating one of the tougher Medicaid environments in recent memory.

That beat hides the real pressure inside Molina's cost structure: GAAP net income fell 52% to $38 million as claims costs absorbed more of every premium dollar. The consolidated medical care ratio rose year over year, and Medicaid, the company's largest segment, saw its own ratio climb as well.

Membership is shrinking, too. Total enrollment fell to roughly 4.9 million from 5.5 million a year ago, with Medicaid and Marketplace both seeing declines. Premium revenue of $10.46 billion dropped about 4% year over year as a result, and while rate updates softened the blow, they did not fully offset lost members.

Management gave investors the clearest reason to sit tight rather than sell. CEO Joseph Zubretsky said the company is seeing an apparent plateauing of medical cost trend and that the strength of 2027 rate updates suggests the company is "likely observing the beginning of the turn in Medicaid margins." Molina reaffirmed full-year 2026 guidance of at least $5.25 in adjusted EPS.

This quarter's beat clears a very low bar set by the loss Molina posted in Q4 2025. Whether 2027 rate relief actually materializes into margin recovery is the question that will drive the stock from here.
