The medical loss ratio improved to 84.0% from 88.5%. Oscar Health now targets share gains and margin expansion in 2027.

Oscar Health swung to a profit of $0.49 per diluted share in the third quarter, a result that upended a Wall Street consensus calling for a loss of $0.17. Analysts were modeling the wrong sign entirely, and the company delivered a meaningfully positive number. Revenue came in at $5.14 billion, clearing the consensus estimate and representing a sharp jump from the same quarter a year earlier.

Two items embedded in those GAAP figures are worth understanding before drawing conclusions. The quarter included a $250 million benefit from updated risk adjustment estimates tied to favorable risk score development relative to the broader market, and $42 million of favorable prior period development. Neither of those is a recurring operating line, so investors will want to weigh how much of the profit swing they explain when modeling future quarters.

The medical loss ratio, which measures how much of premium revenue goes out the door as claims, improved to 84.0% from 88.5% a year ago. That is the metric management has pointed to as evidence of pricing discipline, and a move of that magnitude in a single year is substantial for a health insurer. The SG&A expense ratio also fell, to 14.0% from 17.5%, as fixed costs were spread over a much larger membership base that grew to roughly 2.69 million from about 2.11 million.

Management raised full-year 2026 earnings from operations guidance to a range of $850 million to $950 million, up from the prior range of $600 million to $800 million. This is the third time they have lifted that figure in 2026 alone. Full-year total revenue guidance was also nudged higher, to $19.0 to $19.3 billion from $18.7 to $19.0 billion previously.

Looking into 2027, CEO Mark Bertolini said the company intends to grow market share and expand margins, citing what he described as a rapidly developing ACA individual market. Oscar is flagging several risks that could affect that outlook, including the potential expiration of enhanced Advanced Premium Tax Credits, new ACA program integrity rules, and ongoing uncertainty in risk adjustment estimates. How those regulatory variables resolve will be a central question for the stock heading into the next enrollment season.
