Requisition volume jumped 14.5%, enough to overpower a 4.2% decline in revenue per requisition and push the quarter past estimates on both lines. Management responded by raising full-year adjusted EPS guidance to $11.25-$11.35 and lifting operating cash flow to roughly $1.90 billion. Volume is carrying this story.

Quest Diagnostics beat on both lines in the third quarter of 2026, with adjusted diluted EPS of $3.00 topping the consensus estimate, continuing a streak of quarterly outperformance that stretches back through 2024.

The engine behind the quarter was a 14.5% jump in requisition volume, reflecting demand across physician, hospital, and consumer channels. That surge was strong enough to absorb a 4.2% decline in revenue per requisition and still drive meaningful top-line growth, which is the dynamic investors will want to track in coming quarters.

Profitability improved alongside the volume gains, with the adjusted operating margin expanding to 16.7% from 16.3% a year ago. CEO Jim Davis credited automation and AI-driven productivity gains alongside the channel strength.

Management responded to the quarter by raising full-year 2026 guidance across the board, lifting adjusted diluted EPS guidance to $11.25 to $11.35 alongside higher revenue and cash from operations outlooks. Capital expenditure guidance stayed at approximately $550 million.

Beyond the headline numbers, Quest is advancing several initiatives that extend the volume story: a new Alzheimer's disease test panel under the AD-Detect brand, a partnership with Apple to offer a 50-plus biomarker lab panel through the Apple Health app, and the acquisition of outreach lab assets from a Northeastern health system. An Investor Day is planned for March 25, 2027 at the New York Stock Exchange.
