Tenet Healthcare Q3 2026: A 42% EPS Beat With an Asterisk
As seen on the 24/7 Wall St. homepage on October 29, 2026.
Tenet raised its full-year adjusted EBITDA outlook to $5.05 billion to $5.20 billion after beating expectations on both lines. The quality of that raise matters: $147 million of the adjusted EBITDA gain came from timing tied to the early wind-down of Conifer's CommonSpirit contract.

Tenet Healthcare reported adjusted EPS of $6.64 for the third quarter of 2026, handily exceeding expectations. The composition of that beat is what investors are picking apart.
A $147 million favorable net Adjusted EBITDA impact came from timing tied to the early wind-down of Conifer Health Solutions' revenue cycle management contract with CommonSpirit Health, pulling future adjustments into the current quarter. Stripped of that item, the underlying result is still solid but meaningfully smaller than the headline suggests.
Away from the Conifer timing benefit, USPI's Adjusted EBITDA climbed 9.1%, powered by higher-acuity surgical cases and higher net revenue per case. Hospital admissions also rose, pointing to real volume strength in the core hospital segment.

Tenet raised its full-year 2026 Adjusted EBITDA outlook by $195 million at the midpoint, alongside higher full-year adjusted diluted EPS guidance. The company also repurchased shares during the quarter and reduced its net debt to Adjusted EBITDA ratio to 2.11 times.
The risks worth tracking include declines in ambulatory same-facility surgical cases and hospital surgeries, plus an unfavorable payer mix shift away from exchange admissions. The Conifer unit also faces the structural reality that the CommonSpirit contract, once wound down, removes a recurring revenue stream. How much of the raised guidance rests on durable operating improvement versus one-time timing items is the question the next few quarters will answer.
